Cleaning, Decontamination and Disinfection is the single largest domain on the HSPA CRCST exam — about 21.1% of the scored questions. Process

The decontamination workflow

The exam tests the order as much as the steps. Decontamination runs one way — dirty to clean — and every item passes through the same line. Know the sequence, and know why each step exists.

  1. Transport in, kept contained

    Soiled instruments arrive from the point of use kept moist and enclosed, entering on the dirty side only. Dried-on bioburden is far harder to remove, so the clock starts at the OR, not at the sink.

  2. Sort, disassemble, remove gross soil

    Sort by cleaning method, open box locks, disassemble multi-part instruments and flush lumens. Gross debris comes off before any detergent step so the chemistry can reach the surface.

  3. Manual cleaning below the waterline

    Brush and flush under the surface of the water to control splash and aerosols, using an enzymatic or detergent soak mixed to the instructions for use (IFU).

  4. Ultrasonic for the fine detail

    Cavitation reaches serrations, box locks and hinges that a brush misses — but only after gross soil is gone, or the debris just redistributes.

  5. Mechanical wash-disinfection

    A washer-disinfector runs a validated wash and then a thermal or chemical disinfection stage, giving a repeatable, documented result manual cleaning cannot.

  6. Rinse with the right water

    A final rinse in treated water prevents mineral spotting and chemical residue; water quality is part of the process, not an afterthought.

  7. Inspect and verify

    Magnified inspection plus a cleaning-verification check before anything crosses to preparation and packaging. Clean is proven, not assumed.

Spaulding classification — the 'what level' rule

Spaulding is the spine of this domain. It sorts every device by infection risk and sets the minimum reprocessing level. When a question describes what a device touches, it is asking you to place it on this table.

Chemistry

Cleaning chemistries and water

You do not need a chemist's depth here — you need to match the agent to the soil and mix it correctly. Enzymatics break down protein, blood and other organic soil; detergents lift and suspend it. Water quality sits underneath both: hardness and minerals affect how well a detergent works and whether the final rinse leaves residue.

One-step cleaner/disinfectants tolerate some soil; a plain disinfectant does not — visible matter must be removed first, or disinfection fails.

Safety

PPE, one-way flow and exposure

The decontamination area runs on one-way flow — dirty to clean, never back — and personal protective equipment is chosen by the task, not by comfort. Under the OSHA Bloodborne Pathogens standard, PPE is selected for the anticipated exposure: fluid-resistant gown, gloves and face protection matched to the splash, aerosol and sharps risk of the job in front of you. A high-volume flushing task calls for heavier fluid-resistant material than a light one; you do not remove protection because the room is warm.

Quality

Verifying the clean, and biofilm

Cleaning is checked, not trusted. Beyond visual and magnified inspection, cleaning-verification tests (for protein or other residue markers) confirm the process worked. The exam wants you to read a pattern: a single failed residue test is one thing, but recurring failures across a fleet of scopes point to embedded biofilm — a systemic problem, not bad luck.
